Privacy Statement
Two different things are covered here, and they are worth keeping apart: what this website does, which is almost nothing, and what we do with the information you give us when you come in for a collection, which is the part that actually matters.
Identity

We are a specimen collection site. We collect samples and send them to certified laboratories for analysis. We are not the laboratory, and in most cases we are not the party that issues your result.
Online
This website
This site sets no cookies, runs no analytics, and carries no advertising or third-party tracking. There is no contact form. Nothing you do here is recorded or profiled by us.
Our hosting provider keeps standard server logs, which may include your IP address, browser type and the pages requested. That is ordinary infrastructure record-keeping used to keep the site running and secure. We do not use it to identify or track individuals.
Booking online
Our Book Online button hands you to Square Appointments, a separate company. Anything you enter there, including your name, telephone number and email address, is collected by Square under Square's own privacy policy, not this one. We receive the booking details Square passes to us so we know to expect you.
What we collect when you come in
A collection cannot be done anonymously. Its whole purpose is to prove that a specific sample came from a specific person, so identity checking is not an administrative extra, it is the service. When you visit we typically record:
- Your name and date of birth, verified against current government-issued photo ID.
- The authorization, registration number or court document that sent you to us, where one applies.
- Chain-of-custody documentation: what was collected, when, by whom, and every hand the specimen passed through.
- The specimen itself, which goes to the laboratory under seal.
- Payment details, if you are paying directly. Card payments are processed by our payment provider. We do not store your full card number.
Authorized recipients
Who your information goes to
Results go only to the party authorized to receive them. Who that is was decided before you arrived, by whoever ordered the test. Depending on your situation it may be:
- The laboratory that analyzes your specimen.
- A Medical Review Officer, for federally regulated testing, where a physician reviews the result before it is released.
- Your employer or their third-party administrator, for workplace and DOT testing.
- A court, attorney, probation office or agency, where the test was ordered for a legal matter.
- You, where you ordered the test yourself.
We may also disclose information where the law requires it, such as a valid subpoena or court order.
We do not sell your information. We do not share it for advertising. We do not disclose your results to anyone outside the list above. If your employer sent you, we report to your employer. If a court sent you, we report as the order directs. Anyone else who calls asking about you gets nothing.
Records
How long we keep it
Collection and chain-of-custody records are retained for as long as the applicable regulation, contract or legal requirement demands, and then disposed of securely. Federally regulated testing carries its own retention periods set by regulation, and we follow those.
Keeping it secure
Paper records are held in a controlled site. Specimens are sealed, labeled and tracked from collection to laboratory. Access is limited to trained staff who need it to do the work. No system is perfectly secure, and we will not pretend otherwise, but the chain-of-custody procedure exists precisely so that any interference is visible rather than silent.
Access and correction
Your choices
You can decline to be tested. What follows from that is between you and whoever asked you to test, and it is not a decision we make. We cannot collect without valid photo ID, because a sample nobody can tie to a person is not a usable result.
To ask what we hold about you, or to correct it, call (480) 210-1016 or email Admin@trustscreenlab.com. We may need to verify your identity before we answer, for the same reason we check ID at the counter.
Some requests we cannot grant. We cannot delete a chain-of-custody record that a regulation requires us to keep, and we cannot release another party's result to you.
Children
This website is not directed at children. Minors are tested only with a parent or guardian present and the required documentation, and we hold that information on the same terms as everything else here.
Changes to this statement
If this changes, the date at the top of the page changes with it. The version published here is the current one.
